The Dietary and Lifestyle Phone Support Program is a FREE, evidenced based program, delivered by dietitians. Referrals are welcome from practices in the Logan-Beaudesert area, for patients wishing to make lifestyle changes and lose weight *eligibility criteria apply. Click here to download referral form

If your practice is within the Logan (download division map) or Beaudesert (download list) area, why not speak with your practice manager about becoming a referring site. Once your practice is registered as a referring site, access to the program is simple.
Referring methods:
