The SouthEast Primary HealthCare Network supports individual General Practices to establish Diabetic Clinics and streamline and enhance diabetic care in their practices.
A Primary Care Liaison Officer from SouthEast Primary HealthCare Network will provide resources and support General Practices in the set up of a Diabetes clinic to maximise patient care and clinic profitability. Education and resources on diabetic care planning will also be provided (GP Management Plans, Team Care Arrangements and the Annual Cycle of Care).
We have Credentialed Diabetes Educators that conduct visits to the clinics to increase the capacity of Practice Nurses to manage Diabetic Patients including education about foot care and management, diet and lifestyle modification, insulin, weight management etc.
