The Medication Management Review Program allows GPs to work in collaboration with pharmacists to undertake home medicine reviews for suitable patients. GPs can help improve patient health outcomes by ensuring appropriate medication use and preventing medication-related problems.
Many patients may be taking over the counter and other medications that have not been prescribed by their usual GP. In addition, impaired hearing and sight, problems with dexterity and confusion may add to medicine-related problems resulting in overdose, insufficient dosing, adverse effects and drug interactions.
Home medicine reviews by accredited pharmacists can reduce the risks by helping patients to understand their medicines better and improving their ability to manage their medicines appropriately. Reviews are undertaken in the comfort of patients' homes and a complete written report is issued to the referring GP.
