The SouthEast Primary HealthCare Network is committed to improving the coordination of services through the development of referral pathways and protocols in the primary care sector.

Under resources you will find referral pathways for several conditions, and referal templates to the major Specialists Out Patients Departments for Medical Director and Best Practice, and the Metro South GP Referral Criteria 2001.

These resources are provided for practices to assist in development of quality referrals, to allow appropriate triage and categorization of patients.

Coronary heart disease management in general practice Cardiovascular disease (CVD) remains one of Australia s most devastating health problems, with coronary heart disease the leading single cause of death in Australia. Cardiovascular disease is also the leading cause of death and disability in Queensland. Therefore, reducing CVD is a key to improving the health of Queenslanders. The Heart Foundation is committed to supporting general practice in the adoption of prevention and management recommendations in order to minimise the evidence-management gap for CVD, in particular coronary heart disease, and to reduce the associated disease burden in Australia. A checklist of valuable information and resources is included in this attached PDF that may assist when planning best-practice management and models of care to support programs for patients with, and at risk of developing CVD.

Podiatry - Logan Diabetes Services Referral Pathway The Logan Diabetes Service are now providing Podiatry Services at Logan Hospital and Browns Plains Community Health Centre. This document gives referral criteria and referral pathways information. All referrals will be triaged according to the "Queensland Health Guidelines for Care and Referral of Adults with Type 2 Diabetes - 2006". Should you require clarification regarding this notification contact the podiatrist on 3299 8383 to discuss this further.
